I almost got it right. I've used a ball 'pein' before to roll over the bottom thread.  Blush 

I've only had it happen to me once where a stud worked it's way past a poorly chiseled import t-nut. After that I started buying the Te-Co hardware as well. No burrs and no rounded corners either. Thumbsup
The table on my shaper has a couple of major "blown-out" sections on, which may have been caused by this. I'm not sure how else it could have been done, as I am sure the belts would have slipped before the ram could cause that big of a crash.
Maybe someone used them as lifting points?  Chin 
Possibly - although one is on the top and the other on the side. Perhaps they rotate it 45° and tried to lift it.

Either way - I have no desire to test out how they did it...
